Community Health Partnerships

As head tenant for the NHS LIFT estate, CHP is responsible for the overall management of LIFT buildings across England. Its focus is to support commissioners, CCGs, NHS England, GPs and Local Authorities to plan and utilise their estates efficiently and ultimately drive much needed savings.

Birmingham City Council

Birmingham City Council is the local government body for Birmingham and represents more than one million people across 40 wards and 10 district areas throughout the City. It’s responsible for running a variety of local services, from social care to health and wellbeing.

Solihull Metropolitan Borough Council

Solihull Metropolitan Borough Council comprises 17 wards, with its objectives structured around five key themes - population and demography, prosperous communities, a brighter future for children and young people, stronger and safer communities and healthier communities.

Birmingham Community Healthcare NHS Trust

Birmingham Community Healthcare NHS Trust provides high quality, accessible and responsive community and specialist services within Birmingham and the West Midlands. It delivers in excess of 100 clinical services, in people’s homes and in more than 400 hospitals, health centres and clinics. This includes services for adults, children, people with learning disabilities, those with rehabilitation needs and also dental services.

Birmingham CrossCity CCG

Birmingham CrossCity is a clinically-led organisation with the aim of improving health and health care within local communities and across the city of Birmingham, based on the best available understanding of the health needs of the population.

Birmingham South Central CCG

Birmingham South Central CCG is a commissioning organisation appointed by the Government to design and buy local health services for approximately 250,000 people in the south and centre of Birmingham.

Sandwell and West Birmingham CCG

Sandwell and West Birmingham CCG is a membership organisation involving 110 GP practices serving around 530,000 patients across the Sandwell and West Birmingham area, working to improve the health of the communities it serves and the quality of health and social care services provided to those communities.

Solihull CCG

Solihull CCG covers two localities: Solis, which covers north Solihull, and Sirius, covering central and south of the borough. It is responsible for commissioning innovative, high quality services that bring care closer to people’s homes and make people healthier - reducing inequalities and creating a sustainable NHS, fit for the future.
